Buckinghamshire Healthcare NHS Trust Heart Failure Service aims to provide gold standard investigation and treatment for patients suffering from heart failure, both in hospital and in the community.
The fully integrated service supports the care of those suffering from advanced heart failure both in hospital and in the community. It is staffed by a team of dedicated heart failure nurses, providing heart failure clinics throughout Buckinghamshire and a home visiting service for the housebound.
Our heart failure nurses make frequent visits to patients in our hospitals who have been diagnosed with heart failure symptoms to ensure that they receive seamless care once they are discharged.
The team works closely with our adult community healthcare teams, GPs, cardiologists, practice nurses, palliative care teams, social care services and cardiac rehabilitation teams. The service also links in with other specialist teams for diabetes, chronic obstructive pulmonary disease (COPD), renal dysfunction and the Healthy Minds mental health wellbeing team.
The service helps patients to improve their quality of life by educating and supporting them in self-management of their heart condition and by reducing the impact of symptoms. Support it also provided to families and carers as they play a key role in the day-to-day lives of our patients.
